Responsibilities
Conduct Initial Patient Assessments and Develop Plans of Care Perform comprehensive assessments for new patients, complete OASIS documentation, and develop individualized home health care plans in collaboration with physicians and interdisciplinary team members. Deliver High-Quality Clinical Care in the Home Setting Provide skilled nursing care including education, disease management, medication review, and implementation of physician-ordered interventions while promoting patient independence and safety. Ensure Accurate Documentation and Interdisciplinary Communication Maintain timely and accurate clinical records, participate in quality assurance activities, and communicate effectively with team members, physicians, and caregivers to ensure coordinated, patient-centered care.
Qualifications
Education:
- Graduate of State Board approved program for Registered Nurses
Experience:
- One year nursing experience, including experience in performance of OASIS, in skilled home health care
License(s)/Certification(s):
- Valid license as RN in state(s) where providing care.
- Valid licensed driver with automobile insurance in accordance with state(s) and/or organizational requirements
- CPR: Maintain a valid Basic Life Support (BLS) Healthcare Provider Card with Re-certification.
