At the Telecare Fern Lodge Behavioral Wellness program, we provide intensive mental health and psychiatric treatment services in a safe, welcoming environment for adults requiring continued time for stabilization and recovery support.
We believe recovery starts from within and that our job is to do whatever it takes to provide the support needed on our residents' recovery journeys. Our secure facility includes a full staff of registered nurses, clinicians, peer specialists, and social workers on-call 24 hours a day to provide wraparound support.

Our services are designed using Telecare's Recovery-Centered Clinical System (RCCS). Through the lens of trauma-informed care, this system incorporates evidence-based practices and innovative design components. RCCS focuses on choice-making skills, harm reduction techniques and strives to awaken the hopes and dreams of the individual. The RCCS emphasizes "no-force first" practices. Staff work with individuals within their cultural dynamic in building independence and self-responsibility in order to foster their recovery and successfully transition them home or to a less restrictive community setting.
A variety of therapies and activities adapted to individual needs and preferences are offered as part of their treatment plan. Services at Telecare Fern Lodge Behavioral Wellness include, but are not limited to:



  • Comprehensive evaluation and risk assessment, covering mental health and substance abuse
  • Psychiatric assessment and treatment
  • Peer support and community group meetings
  • Social and recreational activities
  • Rehabilitation programming
  • Supportive counseling on a group and individual basis
  • Medication administration and management

POSITION SUMMARY

The Peer Health Navigator engages and connects members served to mental health, primary care, substance use, and specialty health care services; helps them develop personal health and wellness goals; educates them on how to advocate for themselves and effectively communicate their needs; and supports them in overcoming barriers and building resilience. The Peer Health Navigator supports members served in maintaining a follow-up care plan and developing other self-management skills through a "Modeling - navigator performs task; member served observes, Coaching - member served performs task; navigator coaches, Fading - member served self-directs and manages own healthcare; navigator assist as needed" process. The Peer Health Navigator simultaneously serves as a role model of resilience, recovery, and self-responsibility for their own mental and physical health. They continually promote and reinforce Telecare's Recovery Culture as defined by the Telecare Recovery Centered Clinical System (RCCS) in which the member's point of view and preferences are recognized, understood, respected, and integrated into services and self-help programming. Additionally, Peer Health Navigators will respond to critical situations with high-level engagement and de-escalation skills which support a least restrictive environment for members served experiencing an emergency related to a mental health/addiction challenge.

Shift:

Full-Time 1.0 | Swing | 10:00 am to 7:00pm | Monday - Friday

The expected starting wage range is $21.05 - $25.38. The full wage range goes up to $29.70. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

QUALIFICATIONS

Required:

* High School Diploma or GED required

* Peer Certification must be received prior to the hire date and must be maintained throughout employment

o Washington: obtain Certified Peer Counselor (CPC) through HCA before hire date and maintain throughout employment and be certified by the Department of Health (DOH) as an Agency Affiliated Counselor (AAC) within 60 days of employment.

* Two (2) years' experience or part-time equivalent supporting individuals with behavioral health challenges

* Experience supporting individuals with physical health challenges to navigate the physical health care system

* Experience as a beneficiary of the public Behavioral Health system of care

* Must be at least 18 years of age

* Must be CPR, First Aid, and Crisis Prevention Institute (CPI) certified (hands-on course) on date of employment or before providing direct client care, and maintain current certification throughout employment

* All opportunities at Telecare are contingent upon successful completion and receipt of acceptable results of the applicable post-offer physical examination, 2-step PPD test for tuberculosis, acceptable criminal background clearances, excluded party sanctions, and degree or license verification. If the position requires driving, a valid driver's license, a motor vehicle clearance, and proof of auto insurance are required at the time of employment and must be maintained throughout employment. Additional regulatory, contractual, or local requirements may apply.



  • Valid and current driver's license, and personal vehicle insurance with your name listed as a driver.
  • Willingness to use your vehicle to attend meetings, etc. (weekly mileage reimbursement at the IRS rate)

ESSENTIAL FUNCTIONS

* Demonstrates the Telecare mission, purpose, values, and beliefs in everyday language and contact with the internal and external stakeholders

* Draws upon own personal recovery experience with mental health/addiction challenges as a basis for engagement as a peer with the members served and to validate members' own lived experiences

* Assists members served with voicing their strengths, interests, and goals throughout their time in services, including: goal plans, crisis plan,s and other methods of expressing individual preferences

* Utilizes a comprehensive resource guide and educates members about local community supports and resources

* Supports members served in developing strategies to reduce self-stigma

* Engages and communicates with a wide range of members served, especially those who are struggling to be active in their own recovery

* Assists participants in all need,s including Activities of Daily Living (ADLs)

* Actively participates in multidisciplinary team meetings

* Participates in verbal de-escalation and physical interventions in emergent situations and is willing and able to assist other staff as needed to maintain a safe, healing environment for all

* Travels locally to provide outreach to members served in the community (program-specific)

* If employed at an Inpatient program, must assist with the restraint of members served in the event of assaultive behavior and pass assault crisis/crisis prevention training

* If employed at an Outpatient program, must evade members served in the event of assaultive behavior and pass assault crisis/crisis prevention training

Duties and responsibilities may be added, deleted, and/or changed at the discretion of management.

SKILLS

* Knowledge of recovery-focused, strengths-based work in mental health/addiction recovery

* Willing to seek to understand members' unique circumstances, personal preferences, and goals, and incorporate them into daily work as well as any needed crisis response to help them regain a sense of control

* Have a strong understanding of recovery and resilience, the value of partnerships with members and member choice, and the balance between protection from harm and personal dignity

* Strong knowledge of relevant community resources and methods for accessing them

* Possess strong communication, writing skills, and typing/computer skills

* Experience working with adults facing mental health/addiction challenges

* Interest or experience in working with people who are difficult to engage and maintain in traditional mental health programs

* Ability to communicate and work effectively with members served, family members, and staff from various backgrounds

PHYSICAL DEMANDS

The physical demands here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

The employee is occasionally required to stand, twist, squat, kneel, and lift and carry items weighing 25 pounds or less, as well as to frequently sit, walk, reach, bend, and do simple and power grasping. The position requires manual deviation, repetition, and dexterity.
