The Guidance Center (TGC) is the Community Mental Health Center and Certified Behavioral Health Clinic serving Leavenworth, Atchison, and Jefferson counties in Kansas. The Guidance Center (TGC) is currently seeking an ACT Targeted Case Manager. This individual provides Community Psychiatric Support and Treatment (CPST) and Targeted Case Management (TCM) services to SPMI adults, following the strengths model of case management. Services are provided primarily in the clients' homes, community, or other natural settings. This position has an evenings on call requirement on a rotating basis.

PRIMARY RESPONSIBILITIES:



  1. Under the supervision of a QMHP, the TCM will facilitate the treatment plan process by inviting all involved participants, to include client to develop or update a plan which addresses identifies needs, defines goals and objectives, and action steps which support community living and participation.





  1. Enter treatment plan details into electronic record and other documents in a timely manner.





  1. Monitor treatment plans to ensure the plan is current, ensure that updates are accomplished before the plan expires and enlist assistance of supervisor if problems are encountered.





  1. Provide face-to-face, phone and written communication with caregivers, other service agencies, housing resources, and medical services to assist with meeting client/family needs.





  1. Provide assertive outreach directed toward engaging and maintaining client relationships as well as relationships with caregivers.



EDUCATION AND EXPERIENCE:

Bachelor's degree required or be equivalently qualified by work experience or a combination of education and work experience in the human services field with one year of experience substituting for one year of education.
