The Clinical Pharmacist Patient Care will serve as a key member of the interdisciplinary care team to identify, prevent, and resolve drug therapy problems, improving member health, and decreasing total cost of care. You'll enjoy the flexibility to work remotely * from anywhere within the U.S. as you take on some tough challenges. Primary Responsibilities:
- Support Team, Health Plan, & Leadership Goals
- Build and maintain client relationships and serve as a primary point of contact for clinical needs with each UHC plan
- Develop a solid working knowledge of health plan operations (claims adjudications, case management, HEDIS measurements, etc.)
- Understand company financial goals along with NPS scores and utilize this knowledge to contribute to their success
- Interdisciplinary Care Team Collaboration
- Serve as a key member of the interdisciplinary care team to identify, prevent, and resolve drug therapy problems, improving member health, and decreasing total cost of care
- Collaborate with the interdisciplinary care team to coordinate pharmacy care to members served including but not limited to internal and external MDs, RNs, social workers, community health workers, behavioral health providers, housing coordinators, retail pharmacists, etc.
- Advise team of medication adherence gaps and provide advice to remove any barriers to care and improve medication adherence across our member population
- Clinical Reviews
- Review and advise on medications and treatment plans (based on review of medical case notes, medication list, and lab results) to improve outcomes and avoid potential drug interactions
- Collaborate with the Quality team to review hospital discharge orders and reconcile discharge medications for members transferring home
- Review medication regimen for disease state for LTC facilities / hospice care / RN case managers / medical directors / clients and provide summaries and guidance on future medication plans
- Review medical case notes to optimize ongoing medication treatment plans and project/reduce future medication costs
- Treatment optimization
- Advise prescriber/member on alternate drug therapies, polypharmacy, dose changes, etc. to improve a member's treatment plan
- Advise members on alternate Rx fill strategies including retail, delivery, mail order and options for financial assistance
- May outreach to providers on suspected medication abuse situations
- Program Development and Implementation
- Lead in the design, implementation, and execution of clinical pharmacy programs to improve member care and decrease barriers to pharmacy care
- Continually evaluate clinical programs to optimize their outcomes and streamline the process
- Collaborate across the enterprise to utilize all resources available to maximize the outcomes and total cost of care savings achieved

- PharmD from an accredited college or university
- Current and unrestricted Pharmacist license in their state of residence
- Willing/able to apply and obtain additional state pharmacist licensures per needs of the business
- 5+ years of direct clinical pharmacist experience in a non-retail pharmacy environment
- 5+ years of health plan or Pharmacy Benefit Management experience
- 5+ years of experience/ability pulling reports from various data systems and synthesizing data
Preferred Qualifications:
- Board certification
- Experience working in RxClaims
- Experience working in EMR systems
- Medical benefit experience
- Proficient in PowerPoint and Excel
- Large scale project management across matrix enterprise teams and functions
- Proven analytic acumen; ability to analyze data to interpret clinically impactful insights
- Proven ability to train and adapt to multiple claims systems, clinical dashboards, and reporting tools
- Proven excellent presentation skills to a variety of key stakeholders including but not limited to leadership, health plans, interdisciplinary teams, etc.

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
