Description Maine Medical CenterProfessional - Clinical ~ Per Diem / As Needed Position, Varied shifts ~ Maine state licensure to practice as a Licensed Clinical Social Worker (LCSW) or Licensed Masters Social Worker - Clinical Conditional (LMSW-CC) is required The Barbara Bush Children's Hospital is a 30-bed Pediatric Unit, serving the children in the state of Maine from birth to young adulthood. From routine check-ups and immunizations to the treatment of life-threatening illnesses and injuries, we provide comprehensive, family-centered healthcare for all of northern New England. Here, children receive care that's as unique as they are. As Maine's only full-service children's hospital, we offer everything from well-child visits to specialized medical and surgical treatment of complex illnesses and injuries. We serve the children in the state of Maine from birth to young adulthood. We have over 20 sub-specialists of Pediatric Services. These range from oncology, general surgery, cardiology and many others. Summary The Care Management Social Worker I role is responsible for the ongoing assessment of the biopsychosocial-spiritual status of the patient and family/supports. The social worker will work collaboratively with the interdisciplinary team members, both internal and external to the organization to facilitate discussions regarding discharge options as they relate to complex situations, facility placement, or community resources in the context of the patient's health insurance, available support, location and other eligibility factors.
