As a Clinical Nurse at West Valley Hospital, you are the heartbeat of patient care in a rural community hospital setting. Your work goes beyond tasks-you serve as a vital partner in healing, safety, and recovery. Whether you're delivering hands-on care, guiding families through complex medical situations, or leading with evidence-based practices, your role directly shapes the patient experience and health outcomes.

In this expanding 25-bed department, you'll care for patients with acute, skilled nursing, and observation needs, contributing to a collaborative and highly engaged team that thrives on mutual respect, flexibility, and shared accountability. You won't just be following protocols-you'll help shape them through staff-led quality improvement initiatives and ongoing innovation.

This position offers the unique opportunity to practice full-spectrum nursing-assessment, intervention, planning, and leadership-while building deep connections with patients and colleagues. You'll help ensure every patient receives personalized, high-quality care, and your input will drive continual improvement in a hospital that is small enough to hear your voice, yet big enough to make a real difference.

What you'll do

  • Perform accurate, ongoing, and age-appropriate assessments for assigned patients, including historical, medical, social, cultural, and educational factors. Report and consult pertinent data with the care team and identify patient/family education needs using appropriate language and reinforcement.
  • Deliver patient care based on the care plan, following Salem Health-West Valley Hospital policies and procedures.
  • Demonstrate age-specific care by involving family or caregivers, encouraging self-care, supporting patients through anticipated changes, and promoting reminiscence when appropriate.
  • Prioritize care based on current patient needs, proactively prevent adverse outcomes, and respond appropriately in emergency situations.
  • Develop, review, and update individualized plans of care in collaboration with the patient and family, reflecting changing conditions, rehabilitation potential, and discharge planning needs in coordination with the broader care team.
  • Accurately document assessments, plans, interventions, and patient responses, including teaching efforts, medication and pain management, vaccination status, and discharge notes. Tailor education to the patient's learning needs and involve other team members as needed.
  • Communicate clearly and effectively using the Situation, Background, Assessment, Recommendation (SBAR) model and other tools to share complete, timely information, anticipate needs, provide status updates, and ensure smooth coordination with team members.
  • Demonstrate leadership by applying evidence-based practices, participating in professional development, supporting unit performance initiatives, and engaging in shared decision-making. Serve as charge nurse when assigned, ensuring appropriate staff assignments, supervising care delivery, adapting to changing demands, and addressing issues as they arise.
  • Promote and maintain a safe environment by following infection control policies, using proper lifting techniques, replacing sharps containers, addressing equipment concerns, and keeping hallways clear. Use restraints appropriately and document when necessary.
  • Support resource management through flexibility, cross-training, cooperation, and time management. Maintain supplies, assist in unit operations, and float to other areas when needed to ensure productivity.

Required qualifications

  • Requires a Bachelor of Science (BSN) or Bachelor of Arts degree in nursing, or non-nursing bachelor's degree with a Master of Science in nursing (MSN). Candidates with an associate's degree in nursing (ADN) and graduates of a diploma RN program are also encouraged to apply; ADN applicants with less than five years of acute care experience will be required to sign an agreement committing to complete their BSN or MSN degree within three years of hire.
  • Basic Life Support (BLS).
  • Current unencumbered licensure to practice nursing as a Registered Nurse in the State of Oregon.
  • Minimum of one year of RN Staff Nurse experience required.
