Dementia Care Specialist

Aging and Disability Resource Center (ADRC)

Hourly Range: $31.43 - $37.50

Job Summary: The Dementia Care Specialist provides direct services to persons with dementia and their families with a focus on diverse communities. This position provides culturally appropriate dementia trainings to ADRC staff and the community. The Dementia Care Specialist (DCS) will develop and implement programs to create and sustain culturally appropriate dementia friendly communities and referral protocols with dementia organizations throughout Wisconsin. This position will also expand the reach of the department's dementia programming and will recruit volunteers to assist with programs and events.

Essential Duties and Responsibilities:



  • Train ADRC staff and vendors to be dementia capable as well as train emergency service providers and local businesses to be dementia friendly, ensuring this is done in a culturally responsive manner.
  • Recruit and train volunteers, agencies, business, and interested parties to train community members how to be dementia friendly per state definitions as well as state and ADRC programs designed to assist individuals with dementia or their caregivers.
  • Provide 1:1, in-person consultations with families, caregivers, and persons with dementia including mini cogs, and short-term service coordination. Deliver a minimum of two Department of Human Services (DHS) approved evidence-based/evidence-informed interventions to individuals with dementia and/or their families.
  • Participate in community outreach and public awareness services to provide information, education, and resources related to dementia care, connect individuals to support services, and help develop care plans.
  • Attend conferences, workshops, trainings, and research findings to advance knowledge of dementia.
  • Provide ongoing follow up and support of department initiatives, such as Memory Cafes, Library Hubs, Mini Cogs Administration, program volunteers, intervention programs and like activities.
  • Document outreach activities, referrals in a clear, concise, and objective manner. Maintain 100%-time reports in 15-minute increments.
  • Attend meetings as scheduled and requested with DHS and Milwaukee County ADRC in person and over the phone.
  • Track and report data to writes reports and draft procedures as required by Department of Human Services (DHS) and the Milwaukee County ADRC.
  • Perform other duties as required and assigned.



Minimum Requirements:



  • Must be a current resident of Wisconsin
  • Current valid WI Driver's License (without restrictions), reliable transportation, and insurance both maintained throughout employment.
  • Bachelor's Degree or higher in Human Services, Social Work, or closely related field required
  • At least one (1) year of experience working with clients and/or their families in a human services capacity or closely related field is required.
  • Having flexibility in work schedule, which could include evening hours and weekend hours as needed to meet the clients and family needs.
  • Able and willing to travel up to 60% of work time.
  • Intermediate knowledge of all related computer and software applications, such as word processing and spreadsheets.
  • Intermediate knowledge of case management system and Forward Health system



Preferred Knowledge and/or Experience:



  • At least one (1) year of experience working with patients experiencing dementia.



Knowledge, Skills, and Abilities: Effective speaker before both internal and external groups. Exhibit patience and creative problem solving while working with families. Maintain effective working relationships with persons in stressful caregiving situations. Skilled at recruiting individuals to volunteer their time in service of others. Highly organized and efficient. Work independently while keeping up communication with all parties. Ability to lift, carry, push, pull up to 10lbs.
