General Job Description: The Case Manager, Women's Health Trainer is a lead role within the department, and will conduct extensive training with new and current employees. Will act as a mentor and coach for staff. Process referrals on a first-in first-out basis, schedule patient appointments, obtain patient reports, and provide support services as needed. Job Roles
- Conduct extensive training to staff, and provide coaching and mentoring on an as-needed basis.
- Instruct employees on operational policies and procedures.
- Ensure operational policies and procedures are followed.
- Coordinate work of staff and follow-up on assigned tasks.
- Assist in preparing special reports for management on an as-needed basis.
- Assist management in developing and implementing work improvements.
- Provide input to management in developing measurable goals/performance standards.
- Complete all required documentation accurately, in a timely manner, and thoroughly in accordance with department standards.
- Follow-up on 100% of all positive results for cancer within 72 hours and DETEC cases must, within two months respectively.
- Provide supportive services such as eligibility determination, enrollment assistance, and patient education.
- Track and manage patients to ensure timely care needs are completed, and follow-up care is received as indicated and documented.
- Perform other duties as assigned.
Education/Certifications/Licenses/Registrations
- Bachelor's Degree OR equivalent combination of education and experience that provide the skills, knowledge and ability to perform the essential job duties, and which meets any required state or federal certification requirements.
- Ability and means to travel as needed in a timely manner within San Diego County, to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required. DRIVER REQUIREMENTS: Licensed for a minimum of 3 years; No more than 2 violations and/or accidents within 3 years; No vehicle related suspensions/reinstatements; No DUI, reckless or felony Driving within 7 years.
Experience/Specialized skills (including Language)
- 4 years healthcare-related experience.
- Bilingual in English/Spanish may be required depending on assignment.
- Basic computer literacy ability to comply with department needs and expectations (i.e., electronic medical record documentation, obtaining background information and reports on patients, following up on appointments, etc.).
- Basic organizational skills, attention to detail, time-management skills, and motivation to meet deadlines and achieve goals.
- Knowledge of lead role functions.
- Knowledge of training employees on related job functions.
- Knowledge of leading and directing others.
- Excellent interpersonal and customer services skills with the ability to maintain good working relationships with employees and coworkers.
- Knowledge of data report development.
- Excellent written and verbal communication skills.
- Knowledge of the community resources, health and social service systems in San Diego County and skill in establishing working relationships with community partners.

If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors - we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
