The Aging and Disability Resource Center (ADRC) of Milwaukee County Department of Health & Human Services provides FREE information and assistance regarding community resources and services for adults. ADRC staff assist individuals to plan for short-term and long-term needs, access community services and resources, and learn about programs and benefits available. Promoting Independence and Inclusion. Job Summary: The Community Health Workers are the bridge that connect customers to care services and resources to support their health journey. Community Health Workers are community members who have lived experience in overcoming barriers to access, navigating systems, and utilizing resources in the communities they serve. Knowledge, Skills, and Abilities: Skills needed for success in this role include customer focus, excellent verbal communication skills, patience, ability to write succinctly, professionalism, critical thinking, strong organizational skills, and time management skills. The ability to work collaboratively with other units and organizations within and throughout the county. Excellent credibility in the community. Ability to relate well to people of various ages from diverse ethnic and cultural backgrounds.
