The Patient Navigator provides non-clinical support, guidance, and assistance for patients and families navigating complex healthcare environments. Assists the patient with overcoming healthcare barriers and enhances the patient transfer process within the assigned clinical department to initiate and manage the transfer process, ensuring smooth communication among patients, families, the Transfer Center, and the clinical team members and acts as a communication liaison to understand the patient's non-clinical individual needs and concerns. Guides the patient and family to a broad range of services and resources across the care continuum and works as part of a multidisciplinary team to optimize care and outcomes.
Emergency Department Patient Navigator Job Responsibilities:
- Reviews with the patient, their family, and/or caregiver services available within the program and reviews support staff roles. May screen patients, families, and/or caregivers utilizing evidence-based screening tools for distress and barriers to care and coordinate with the professional staff.
- Supports patient flow throughout the department while partnering with clinical team members to support the department's flow needs.
- Coordinates with the Transfer Center by confirming transport arrangements with the Transfer Center once a bed is assigned.
- Communicates with patients and families by providing regular updates throughout the transfer process to reduce patient and family anxiety and ensure transparency. Closes communication loops by participating in the transfer review calls to ensure all outstanding transfer cases are managed appropriately.
- Identifies patients' logistical needs and assists patients with referrals, i.e., appointments, transportation, child/elder care.
- Works with the professional health care team to discuss patient progress, barriers, and concerns, and discusses plan of care.
- Contacts the patient under the direction ofthe nurse or social work supervisor to review appointments, referrals, and additional resources needed by the patient, their family, and/or caregiver.
- May assist patients with financial resources to help pay for their healthcare or make appropriate referrals to social work or a financial assistant, and may gather financial information for the professional staff.
- Assists patient with specific medical history forms, assessments, referrals, consents, and insurance authorizations.
- Ensures the patient is ready for transport and all forms/required documentation have been completed.
- Coordinates medical interpreters for appointments and office visits.
- May perform other duties as assigned.
Emergency Department Patient Navigator Requirements: Education: Associate's degree in a health-related field OR a HS diploma/GED, with two additional years of experience Experience: Four years of experience in a healthcare setting or an Associate's degree in a health-related field with two years of experience, OR a bachelor's degree in a health-related field with no experience. Certification: Basic Life Support Within 3 Months
