Position Summary

The Chief of Population Health and Evaluation will oversee and lead the population health strategy and initiatives within Mile Square Health Center at The University of Illinois Chicago Hospital and Health Sciences System, a dynamic Federally Qualified Health Center (FQHC) in Chicago. This key leadership role will be responsible for advancing research and population health to support the center's mission to provide high quality, comprehensive, and culturally competent care to underserved populations. The role involves developing, leading and supporting strategies to improve health outcomes, leading data analysis to assess program efficacy, and ensuring that all initiatives align with regulatory requirements and quality standards. The Chief of Population Health and Evaluation works with the quality team, behavioral health and clinical innovations team, clinician scientists, and senior leadership to identify research priorities. The Chief of Population Health role leads in writing grants and securing funding to support research priorities to advance the mission of the health center.

Duties & Responsibilities
  • Lead the development, implementation, and evaluation of the Mile Square Health Center population health strategies, ensuring alignment with organizational goals and community health needs.
  • Provide oversight into initiatives focused on improving health outcomes for high-risk and vulnerable populations, including patients with chronic conditions, behavioral health challenges, and social determinants of health.
  • Collaborate with the Executive Leadership Team and clinical leadership to integrate population health initiatives into the overall healthcare delivery model.
  • Serve as a key advisor on health equity, public health trends, and best practices in population health management.
  • Oversee the collection, analysis, and reporting of population health data to measure the effectiveness of health programs and initiatives in collaboration with the clinical innovation team and QI team.
  • Develop key performance indicators (KPIs) and other metrics to assess program success, patient outcomes, and health disparities in collaboration with the clinical innovation and QI team.
  • Use data-driven insights to inform continuous improvement of health programs, ensuring that they are aligned with evidence-based practices and patient-centered care.
  • Ensure compliance with federal, state, and local regulations (ie, HRSA, IDPH, CDPH) related to population health management and reporting.
  • Supervise the design, implementation, and ongoing management of population health programs, research grants and interventions.
  • Monitor program performance to ensure timely, high-quality delivery of services that meet patient needs and regulatory standards.
  • Promote a culture of continuous improvement by identifying opportunities to streamline operations and enhance program efficiency and effectiveness.
  • Develop and sustain strong partnerships with local health departments, community organizations, healthcare providers, and other stakeholders to promote population health initiatives.
  • Engage in community outreach and education efforts to raise awareness about health resources and promote wellness in underserved populations.
  • Participate in local, regional, state and national-level health policy discussions related to population health, health equity, and access to care.
  • Manage a multidisciplinary team, including program managers, analysts, and other staff working on population health projects.
  • Provide mentoring, coaching, and professional development opportunities for MSHC research engaged faculty, staff, and team members to enhance their skills and capabilities in population health and evaluation.
  • Foster a collaborative, results-oriented environment focused on achieving shared goals for improved patient outcomes and community health.
  • Develop, implement and evaluate community and or lay population engagement in research (eg, citizen scientists, patient navigator etc).
  • Develop, implement and evaluate training for junior scientists (including clinician scientists).
  • Support the division of clinical innovation and collaborate to develop proposals and grants to meet the needs of MSHC patients and communities.


Qualifications:

Minimum Qualifications

  • Doctorate (PhD, DrPH, Pharm D, etc) with minimum 5 years of experience.
  • At least 7-10 years of experience in public health, population health management, or healthcare program management, with a focus on data analysis and quality improvement.
  • Significant experience working in or with Federally Qualified Health Centers (FQHCs) or safety-net healthcare settings is strongly preferred.
  • Proven track record in developing and leading successful population health initiatives, particularly within underserved or diverse communities.
  • Experience with health data systems, electronic health records (EHR), and population health management software.
  • Knowledge of federal and state regulations and reporting requirements related to FQHCs and population health.


Preferred Qualifications

Master's degree in business, law, administration, healthcare, or a related field preferred
Certification in healthcare compliance
Certification in privacy compliance
Prior compliance leadership role within an academic medical center
