Preceptor Home Health & Hospice is growing!
We are a home health, hospice, and palliative care agency providing services in patient homes in 15 counties throughout Wisconsin. Patient homes include private residences, assisted living facilities, memory care units, any place they call home. Preceptor strives to provide high quality care through our multifaceted programs. We have branch offices located in Germantown, Fond du Lac, Waukesha, Sheboygan, and Appleton.
We are currently in need of a Full Time
Director of Clinical Services for our Hospice Department
Offering up to a $5,000 sign on bonus
MINIMAL ON CALL IS REQUIRED

Responsibilities:


  • Oversee the clinical operations of our hospice services ensuring adherence to resident rights and high-quality care standards.

  • Develop, implement, and monitor policies and procedures related to clinical services in compliance with regulations.

  • Manage budgets effectively to ensure financial sustainability while maintaining quality service delivery.

  • Collaborate with interdisciplinary teams to enhance patient outcomes and streamline medical record management.

  • Provide leadership and guidance to staff promoting professional development and ensuring adherence to best practices.

  • Conduct regular audits and assessments of clinical services to identify areas for improvement.

  • Ensure compliance with Medicare regulations and other applicable healthcare standards.

  • Serve as a liaison between residents, families, and healthcare providers to address concerns and improve service.



Job requirements


  • Bachelor's degree in nursing or a related field; master's degree preferred.

  • Valid nursing license in the State of Wisconsin.

  • Proven experience in hospice care with a strong understanding of resident rights.

  • Demonstrate ability in budgeting and financial management within a healthcare environment.

  • Strong leadership skills with the ability to manage diverse teams effectively.

  • Knowledge of medical records management systems and compliance requirements.

  • Excellent communication skills, both verbal and written, with a focus
    on patient centered care.


  • Ability to work collaboratively in a fast-paced environment.
