Behavioral Health Navigator, Primary Care, Merrimack Valley

Primary responsibilities to include:
*Engages patients in the management of their behavioral health conditions through goal-setting, coordination with community services and providers, and referral services.
*Partners closely with clinical social workers and other members of the clinical team to support care plan
*Identifies and removes barriers that interfere with patient access to behavioral health resources.
*Handles follow-up contact with patient/family, care team, and community agencies.
*Promotes an environment that is sensitive to the culturally diverse needs of patients, families and employees.
Research resources and programs available to patients, including but not limited to:
*Individual, group, and family therapists,
*Social opportunities and peer support,
*Behavioral Health Self-Management Tools
As a member of the Care Coordination Team:
*Advocates for the patient to ensure quality outcomes.
*Participates in the planning of care for assigned patient population.
*Identifies and uses appropriate community resources to optimize patient outcomes.
*Finds opportunities for service line process improvement.
*Serves as a resource to the interdisciplinary team for community based social service agencies.
*Maintains effective working relationships to work collaboratively.
*Communicates and collaborates with team to ensure that care coordination needs of assigned patients are met without redundancy of efforts and there is adequate documentation in the patient's medical records.

  • Bachelor's degree.
  • At least 2 years of related work experience required. Experience in human services, social services, and/or behavioral health care is preferred.
