Hospice of Marion County, a member of Empath Health, is seeking a Palliative Care Coordinator to join our team!

ESSENTIAL JOB RESPONSIBILITIES:

  • Acts as a contact person with physicians, internal department staff, hospital personnel, outside agencies, and patient/families to complete referral process

  • Assist Practice Operations Manager to build required reports and distributes monthly.

  • Completes other external surveys and reports as directed.

  • Works collaboratively with the Practice Operations Manager, and practitioners to ensure all functions are completely timely and accurately.

  • Accountable for the accuracy of palliative care data base, all patient data and consultation flow of information for palliative care program.
  • Communicates with insurance companies' representatives as needed.
  • Enters appropriate information in computer program to begin patients' chart and accesses appropriate software systems, finance and other HMC software profiles of referred palliative care patients.
  • Assists with tracking all referrals, Palliative care consultations, admission, non-admissions, as well as transfers to HMC and final disposition.
  • Assist with audits and maintaining accurate records and logs of all referral data and transmits updates monthly to Practice Operations Manager.

QUALIFICATIONS REQUIREMENTS:

  • High School Diploma or GED required.
  • Three (3) years of clerical experience in medial setting required; One (1) year medical record preferred.
  • Highly proficient in working in MS Word, Outlook, Excel and able to quickly grasp functionality/utilization of clinical software programs including EClinical, Netsmart, and others used by various hospitals.
  • Ability to work with all psychosocial groups in a professional manner. Demonstrates tolerance to work with different individual lifestyles without imposing his/her own values on the patient/family unit.
  • Ability to assess, respond to and cope with emotional stress of patients/families, hospital and skilled nursing facilities.
  • Ability to work with minimal supervision, exercising discretion and independent judgement.
  • Ability to read, and interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
  • Ability to write routine reports and correspondence.
  • Ability to speak effectively before groups of customers, patients/families or employees.
  • Knowledge of medical terminology and ability to demonstrate essential usage.
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
  • Ability to use facsimile, copy machines, computers and other common office machines.
