  • Responsible for the assessment, re-assessment, treatment of patients in assigned areas, with maintenance of all records pertinent to treatment. This includes the delivery of physical therapy services, participating in interviews.
  • Evaluates patients assigned for treatment. Completes the initial evaluation and report, including setting short and long-term goals, treatment and documents patient's report in his/her medical record. Renders treatment as determined.
  • Re-evaluates at appropriate intervals and makes the necessary modifications.
  • Maintains records of patients' progress and renders written reports at established intervals.
  • Establishes and maintains treatment schedule for assigned patients; attends conferences, meetings and program rounds.
  • Consults with medical staff regarding recommended therapy services and attends evaluation conferences and makes rounds to discuss progress of patients and problems relating to therapy.
Qualifications
  • Current NY State license to practice Physical Therapy.
  • Graduate of an accredited Bachelor's program in Physical Therapy. Master's Degree is preferred.
  • 2 years of experience preferably in an acute inpatient and outpatient Rehab Medicine Facility.
  • Highly organized individual.

Per Diem Rate = $65 per hour
