Responsibilities

The Perinatal IOP Social Worker provides services within the Perinatal Intensive Outpatient Program (IOP) service line for individuals with psychiatric needs. Our Perinatal IOP services are primarily offered virtually, however all of our team is based at the program location, in person. In this role you may be asked to occasionally help in other non-perinatal programs onsite.

Responsibilities and Accountabilities:

  • Group Facilitation: Lead daily group sessions, including Process Groups and Psychoeducation Groups, to create an engaging and supportive environment for patients.
  • Admission and Intake: Conduct comprehensive admission and intake meetings with new patients, ensuring a smooth transition into the program.
  • Diagnostic Assessments: Occasionally perform diagnostic assessments, including safety evaluations to prioritize patient well-being.
  • Case Management: Collaborate with the treatment team to develop and maintain the Master Treatment Plan, and the development of safety plans, ensuring it aligns with each patient's needs and goals. Participate in weekly rounds with treatment team to discuss patient progress and program success. Work collaboratively with program therapist, assistant therapist/care coordinator and psychiatrist.
  • Patient Engagement: Hold weekly meetings with patients to discuss their progress, review treatment team recommendations, and provide referrals for aftercare based on those suggestions. Address group expectations and rules. Trouble shoot any challenges group members have in attending group.
  • Ongoing Support: Maintain regular phone contact with patients to offer continuous support and encouragement throughout their treatment journey.
  • Grow our program: Partner with the care team and operations team in building additional programming opportunities

Qualifications

  • Master's Degree in Social Work with licensure in the State of Minnesota required.
  • Experience in healthcare setting preferred
  • A passion for and interest in working with the perinatal population
  • Experience using electronic health records preferred
  • Proficient in Microsoft Office Products including Word and Outlook
  • Able to effectively and efficiently communicate in written and verbal form
  • Ability to smoothly and rapidly shift between dissimilar tasks
  • Ability to communicate well both verbally and in writing
  • Ability to react quickly and calmly as part of a team to handle handle safety assessments and emergency situations

Compensation for this role ranges from $29-$34 per hour depending on education and experience.
