Job Title: Clinical Specialty Pharmacist

Part Time 1st Shift

Job Description:

Clinical Pharmacy Specialists are responsible and accountable for the provision of safe, effective, and prompt medication therapy. Through various assignments within the department, they provide support of centralized and decentralized medication use systems as well as deliver optimal medication therapy to patients within their specialty. All pharmacists are responsible for drug preparation and distribution for medications; evaluating, inputting and verification of medication orders; assisting providers and other health care professionals by serving as a drug information resource; monitoring and managing medication regimens per protocol or per consult; and managing pharmacy inventory. Heshe may perform other duties, both major and minor, which are not mentioned below, and specific functions may change from time to time.

  • Works collaboratively with physicians and other members of the healthcare team to develop programs/guidelines that improve practice and patient outcomes.
  • Communicates with a high level of skill to interact with physicians, R.N.'s, patients, and other healthcare providers. Ability to problem solve.
  • Provides verbal and written consultations concerning medication therapy daily.
  • Provides education to patients, physicians, nurses, and other healthcare team members as requested.
  • Initiates, documents, and participates in department activities that promote positive patient outcomes. These activities may include, but are not limited to: MUE, ADR reporting, resolving drug therapy questions, therapeutic drug monitoring, identifying drug interactions and allergies, direct contact with prescribers to clarify major problem orders, and non-formulary drug requests.
  • Provide timely, accurate and effective documentation of clinical pharmacy services.
  • Demonstrates an understanding of oncology disease states and treatments.
  • Can adjust doses of chemotherapeutic drugs based on changes in patient BSA, renal function, complete blood count or other clinical situations.
  • Can recommend antimicrobials for the febrile neutropenia patient.
  • Provides patient education regarding chemotherapeutic regimens.
  • Provides renal, IV to PO, and pharmacokinetic monitoring services.

Requirements:

  • Graduate of an accredited college or university with a Pharmacy degree.
  • Licensed Pharmacist with the Kentucky State Board of Pharmacy in good standing.
  • Pharm.D. or relevant clinical hospital pharmacy experience is desirable.
  • Must provide documentation of state licensure and CE as required by the state of KY.
  • Maintain BLS & ACLS certifications
  • Advanced clinical training or PGY1 residency preferred
