The Disease Management Nurse will perform telephonic/telehealth disease management to educate members to better understand their illnesses, to self-manage their conditions, maintain a healthy diet, and prevent hospitalizations or rehospitalizations. This responsibility is fulfilled to monitor members' compliance with current disease specific clinical guidelines, educate members and caregivers on the specific disease processes, provide ongoing assessments, monitoring, re-evaluation of health-related needs and coordinate care among providers, members, and the community during the Disease Management Program enrollment.

  • Requires a NYS Registered Nurse license and registration.

  • Minimum of two (2) years clinical experience required.

  • Disease management and Managed Care experience preferred.

  • Bilingual Spanish preferred.

  • Knowledge of physical assessment and management of acute and chronic diseases.

  • Strong critical thinking and problem-solving skills.

  • Strong verbal and written communication skills.

  • Ability to function with a high degree of autonomy.

  • Ability to multitask, analyze situations and respond appropriately.

  • Must possess an awareness of and sensitivity to cultural diversity and the various socio-economic levels of members and caregivers.

  • Substantial interpersonal skills necessary to interact with members, their families, and to collaborate with health team members.

  • Knowledge of Medicare and Medicaid regulations.

  • Computer skills: Microsoft Word, Excel, UAS-NY, and Care Management software.
