Description
At Healthcare in Action, we serve patients experiencing homelessness through an innovative "street medicine" approach by contracting with Medicare and Medicaid health plans as well as hospitals and health systems. Street medicine entails using a medically equipped van to deliver medical care directly to people experiencing homelessness in locations such as parking lots, encampments, and parks. The Temporary Referral Coordinator plays a crucial role in ensuring the smooth operation of various tasks related to patient data management, communication, tracking, and specialty referrals. The role involves meticulous data entry, efficient response to team inquiries, proactive tracking and monitoring, and active involvement in special projects as assigned. **This position is remote for a 3-4 month contract, M-F shift 8am-5pm; It is preferred to have familiarity with the Inland Empire region (San Bernardino or Riverside Counties) ** Responsibilities include:
