We are currently seeking a Minimum Data Set (MDS) Coordinator to join our team following a valued team member's retirement. This is a highly collaborative role within a supportive, patient-focused team where your attention to detail, regulatory knowledge, and clinical insight will be highly valued. As a MDS, you will be responsible for the following job duties and responsibilities:



  • Develop, implement and evaluate resident assessment care plans with collaboration of other health professionals to optimize patient well-being.
  • Assessing and monitoring patient health to meet CMS, MDS/RAI, state and federal compliance requirements.
  • Oversee the completion of an MDS that is comprehensive, accurate, standardized and reproducible.
  • Coordinates the completion of Discharge Tracking Forms and Return Tracking forms.
  • Reviews and distributes Quality Indicator data and other reports to improve MDS accuracy and compliance.
  • Attends IDT meetings per facility schedule.
  • Provide educational training related to Medicare, MDS/RAI process to staff as needed.
  • Performs other duties as assigned.


FTE: 0.9; 36 hours per week

Work Schedule: Monday-Friday; Day shifts

Work Environment: Hybrid

Salary: $35.66 - $53.49 hourly

Required Qualifications:



  • Current Minnesota RN or LPN License
  • Maintains current CPR certification
  • Previous MDS/RAI work experience

Benefits are available for non-union staff working 24 hours (0.6 FTE) or union staff working 30 hours (0.75 FTE) per week and are effective the first of the month after 30 days of employment.
