Responsibilities

River Oaks Hospital, located minutes outside of New Orleans, Louisiana, is a 126-bed psychiatric facility that has provided quality treatment to individuals suffering from mental illness and/or addictive disorders since 1970. Situated on a 14-acre campus, inpatient services are available for adults and adolescents. We also offer a partial hospitalization program (PHP) and intensive outpatient programs (IOP) for adults.

The New Orleans Institute provides specialized services for the treatment of trauma and compulsive behaviors. The Eating Disorders Treatment Center offers a comprehensive program for male and female adults with anorexia, bulimia, and related eating disorders. Both the New Orleans Institute and the Eating Disorders Treatment Center are nationally recognized for their excellence. These programs receive referrals from around the United States and are acknowledged as leading centers in their respective fields.

River Oaks Hospital is seeking a dynamic and talented Care Navigator .Ensures the verification, communication, and data collections of patient eligibility for PHP/IOP/ECT or outpatient services upon inpatient discharge.

Manages the workflow of the step-down process by ensuring eligible patients are identified and engaged in PHP/IOP/ECT discussion as a next step in treatment prior to inpatient discharge,

ensuring all services are administered within hospital standards and guidelines; working cooperatively with other patient care personnel while maintaining a high level of quality service to all patients found within the scope of services. Performs all work in accordance with the mission, vision and values of River Oaks.

Job Duties may include but are not limited to:

  • Collects data and generates various reports about the step-down process for the clinical director, CFO, CEO, and other involved departments.

  • Enters collected eligibility criteria into Midas through routine data submission by eligibility, referred, attended, and reason for refusal. Reports this information to the Clinical Director weekly and monthly.

  • Submits data to the PI committee and periodically reviews step-down navigation for process improvement.
  • Attends treatment team staffing's to discuss eligibility and referrals.
  • Meets with the patient on the first day of their step-down to ensure all questions are addressed and the patient and family knows what to expect of the Next Step in their recovery.

Qualifications

Requirements:

  • Education/Experience: High School Education. Experience in inpatient or outpatient mental health setting preferred.
  • One to three years experience working with patients with addiction and or psychiatric issues.
