The position supports the Neurovascular Stroke Center leadership including Medical amp; Surgical Directors and Associate Director. The stroke program coordinator will provide direction to the Stroke Center to ensure quality, timeliness and effective stroke care. This position will be instrumental in outreach activities, coordination of services to patients, and coordinating the activities of the interdisciplinary care team and ensuring readiness for TJC certification. This position will facilitate development of an integrated care model for stroke patients from ED to outpatient services. Program Coordination 45%Assess, plans, monitors and evaluates the program of care for the stroke population. Acts as a resource for stroke education, collaboration and the development and maintenance of standards and practice innovations for stroke. Rounds daily with the stroke team to ensure plans of care are complete to meet necessary core measures Communicates effectively with other members of the patient care team as related to patient needs, plan of care and response to the treatment plan. Acts as patient advocate and liaison with services across the continuum of care. Mentors team members to maintain appropriate documentation of patient care. Provides leadership in design and delivery of effective progress on care management of stroke patients. Identifies areas of opportunities and arranges, conducts team meetings when necessary. Recommend and evaluate processes to improve systems and patient care across the continuum of care. Coordinate mandatory and supplemental education for stroke team members and staff in units involved in stroke care. Coordinate expansion/enhancement of patient and family education materials. Develop and maintain tracking method for patients that are not monitored for core measure or Get with the Guidelines Attends regular meetings with the Central Ohio Trauma Society for guidance in stroke care for emergency medical providers as well as collaborative initiatives with other health systems involved in COTS regarding stoke care. Collaborates and coordinates stroke care with high risk cardiac populations including patients with LVAD and TAVR implantation to ensure quality stroke care and assist with PI plans for special populations of patients Collaborates and provides assistance to UH-E as part of the Telestroke Program and with their Certification activities Quality Improvement/Joint Commission Compliance 35% Works with Medical Directors and team members on quality improvement initiatives. Reviews and maintains policies and procedures specific to the Neurovascular Stroke Center. Maintains the Neurovascular Stroke Center PI Plan in conjunction with the Quality amp; Operations Improvement Department. Works with Quality amp; Operations Improvement to maintain accurate data in the Get with the Guidelines tool for the initial 8 measures and the additional 8 measures being collected Collaborates with Quality and Operations Improvement to address missed measures timely Accountable for development of performance improvement and action plans. Performs regular chart audits to ensure PI plans and action plans continue to be effective Understands GWTG Stroke and data collection methodologies specific to the stroke program. Assists with patient education beyond required stroke education as appropriate and necessary Attends meetings with the Ohio Department of Health Coverdell project to ensure quality care at OSUWMC remains consistent with other stroke centers in the state Maintains, edits and assists with data entry into multiple data bases beyond GWTG to monitor quality of stroke care and patient outcomes Perform discharge phone calls Community Outreach amp; Marketing 15% Collaborate with physicians and health educators to provide stroke education in the community. Participate in regional programs and initiatives in relation to stroke care. Works with marketing/communications to create awareness, messaging and education about stroke. Other duties as assigned-5%May be asked to cover for similar positions during times when co-workers are ill or on vacation. Minimum Qualifications Bachelors degree in nursing or related field, RN with active Ohio license with a special interest in vascular disease and stroke care; masters degree preferred or additional certification or experience commiserate with position requirements; candidate with 5+ years of administrative experience including management, program development and quality improvement initiatives; requires experience with The Joint Commission Primary and or Comprehensive Stroke Center certification. Experience collaborating with and or educating Emergency Medical Service Providers preferred |
