PSG is a community-based social services organization looking for talented mental health and social services professionals like you to join our team. We are seeking dedicated individuals to fill the role of Service Facilitator within our Jefferson County CCS Service Facilitation program.

The Service Facilitator provides intensive case management services to children and adults with severe emotional disabilities (SED), who are at risk of psychiatric inpatient hospitalization or other out-of-home placements. The CCS Service Facilitation program uses the wraparound model of service delivery to identify needed services and develop multi-disciplinary treatment plans that address both the strengths and needs of the family. As a Service Facilitator, you will collaborate with case managers, family advocates, mental health providers, AODA providers, school support staff, juvenile justice workers, and child welfare workers to ensure our clients receive the best possible care.

KEY RESPONSIBILITIES OF THE SERVICE FACILITATOR:



  • Create individual treatment plans and update as changes occur.
  • Develop a family support team comprised of formal and informal supports.
  • Facilitate team meetings with service providers, family members, and family supports in which progress of goals and services are monitored.
  • Develop a crisis/safety plan for the client and family.
  • Exercise judgment to intervene as an advocate for clients in emergency and crisis situations.
  • Attend client/family IEP meetings, psychiatry appointments, court proceedings, or other appointments that may require family support or advocacy.
  • Safely transport clients as needed.


SALARY: $23.00-25.00 per hour for applicants with a Bachelor's Degree; $26.00-29.00 per hour for applicants with a Master's Degree

LOCATION: Position is based at our Jefferson office. Local travel is also required to meet with clients within the home and other community spaces, with mileage reimbursement. Must be able to meet in-person with clients in Jefferson County, Wisconsin.

SCHEDULE: Monday-Friday between 8:00am-5:00pm, which occasional evenings.

REQUIREMENTS:


  • Must have reliable transportation on a daily basis, a valid driver's license, and automobile insurance.
  • Bachelor's Degree in Social Work, Psychology, Criminal Justice, or other related fields.
  • One year experience working with the severely emotionally disabled population is required.
