Compensation:

We offer our employees a competitive compensation and benefits package that reflects our organizational culture, mission, and core values. The hourly range for this position is $23.54 to $24.89 and is commensurate with experience.

We're looking for a compassionate, resourceful Case Manager to join our integrated primary care team at our Federally Qualified Health Center (FQHC). This part-time role (Monday-Friday, 8 AM - 12 PM) is designed to meet the needs of our adult clients, with a focus on clients experiencing unstable housing, multiple medical conditions, and barriers to accessing care.

As the Case Manager, you'll provide hands-on support, helping clients navigate social services, apply for benefits, access housing resources, and connect with medical and behavioral health care. You'll play a crucial role in our morning clinic flow, assisting walk-in clients, conducting intake assessments, and ensuring they receive timely and coordinated care.

This position requires strong communication skills, cultural humility, and the ability to engage clients who may be distrustful of the system. You'll work closely with our Behavioral Health Consultant and medical team to provide client-centered, trauma-informed case management that empowers individuals to improve their health and stability.

Position Description: The Case Manager conducts client intakes and provides comprehensive case management support within the Federally Qualified Health Center (FQHC). This role ensures that clients receive the resources, care coordination, and support services needed to improve their overall well-being.

Schedule: Monday - Friday, 8:00 am - 12:00 pm (20 hours per week) - Integrated Primary Care Clinic

Required: HS Diploma/GED; 3-5 years experience providing case management

Required License/Certification: Basic Life Support [CPR & AED] Provider

Expected Contributions:



  • Strong understanding of mental health conditions, substance use disorders, and their impact on individuals and communities.
  • Highly organized with the ability to manage multiple tasks efficiently while maintaining accuracy.
  • Familiarity with local resources, agencies, and support networks to connect clients with necessary services
  • Skilled in conducting client assessments, developing care plans, and coordinating services
  • Excellent verbal and written communication abilities to effectively engage with clients, colleagues, and external partners.
  • Demonstrated capacity to establish and uphold ethical and professional relationships with clients.
  • Ability to prioritize tasks and meet deadlines in a fast-paced environment.
  • Capable of handling competing priorities while remaining flexible and solution-oriented.
  • Maintains a high level of professionalism and remains calm under pressure in challenging situations.
  • Strong analytical skills to assess situations and develop appropriate solutions.
  • Skilled in using Microsoft Office Suite (Word, Excel, Outlook) for documentation and communication.
  • Ability to communicate and engage effectively with diverse populations.


Knowledge/Skills/Abilities:



  • Provide comprehensive case management services to clients within the Federally Qualified Health Center (FQHC).
  • Offer support to walk-in clients by completing intake assessments and identifying their needs.
  • Evaluate client situations and connect them with appropriate support services and resources.
  • Assist clients in obtaining essential documents such as birth certificates, Social Security cards, insurance, DC Medicaid, and other necessary community resources.
  • Conduct housing assessments and facilitate referrals using the coordinated entry tool (VI-SPDAT) to ensure appropriate housing support.
  • Support clients in applying for public benefits, including food assistance (SNAP) and Social Security benefits.
  • Manage clothing intake and distribution, as needed, to meet client needs.
  • Maintain accurate and timely documentation by completing encounter notes within 24 hours of client sessions.
  • Collaborate with clinicians and service providers to link clients with medical, dental, and behavioral health services.
  • Conduct outreach efforts through phone calls to monitor client progress and ensure compliance with recommended services.
  • Assist clients in securing safe and stable housing by providing guidance and referrals to housing resources.


Reports to: Behavioral Health Consultant

Position Designation: This position is designated as Safety Sensitive. You may be subject to drug testing prior to or during your employment with SOME. In this position, you may be disqualified from employment based on the presence of marijuana in test results, even if you possess a medical card authorizing the use of medical marijuana.

Physical Demands: Must be able to lift to 20 pounds. Must be able to travel off-site using his or her own transportation methods. Requires looking at a computer screen for several hours a day. May be required to sit for long periods. This position may be active and requires standing, walking, bending, kneeling, stooping, crouching, crawling, and climbing.
