Job Details
The Community Health Worker (CHW) is responsible for the implementation of referral strategies to proactively identify appropriate patients for Diabetes Self-Management Training (DSMT) that will help patients form good habits and improve overall health and quality of life. In this position, the CHW will have the opportunity to be part of an exciting new multi-disciplinary team that will make a positive impact in healthcare across the continuum of care.
EXPERIENCE/REQUIREMENTS:
Salary Range: Minimum 20.22 per hour - Maximum 33.38 per hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
