Summary
Responsible for community outreach, engagement, and support to enhance access to behavioral health programs. The Outreach Worker collaborates with community partners, healthcare professionals, and social service agencies to promote mental health awareness and address the behavioral health needs of individuals. Does this position require Patient Care? Yes Essential Functions -Conduct proactive outreach within the community to identify individuals in need of behavioral health services. -Establish relationships with community organizations, schools, primary care providers, and other relevant stakeholders to enhance outreach efforts. -Conduct initial assessments to identify the behavioral health needs of individuals. -Provide information and referrals to appropriate behavioral health services and resources. -Respond to crisis situations and provide immediate support and intervention. -Collaborate with crisis intervention teams, law enforcement, and emergency services as needed. -Advocate for individuals with behavioral health needs to ensure they receive timely and appropriate care. -Conduct community education and awareness programs on mental health topics.
