Assume the role as the expert clinical resource for clinical staff and healthcare providers in the inpatient setting to include support groups. You will plan and coordinate education and care of patients/persons with diabetes across a continuum in collaboration with other disciplines, community agencies and Sentara subsidiaries. You will provide patients and their families staff education to promote wellness and prevention of complications associated with diabetes. Minimum Qualifications License / Certification Registered Nurse (RN) Compact license or Virginia license in good standing. Certified Diabetes Care and Education Specialist (CDCES) required within 2 years of hire. Experience 2 years working with diabetic patients Expert skills in diabetes patient education/care. Ability to perform all interventions and procedures necessary to care for diabetes patients.
