The Discharge Planner is proficient with Medicare, Medicaid, and Commercial Insurance rules/regulations regarding placement/post hospitalization. This job requires knowledge of appropriate resources (community) for post hospitalization care. This job requires a critical thinker able to work independently while also effectively interacting with others. This job requires superior people skills in stressful situations. This job also requires excellent oral/written language competency and computer proficient with typing.
