UVA Physicians Group is seeking a skilled and innovative Ambulatory Care Clinical Pharmacist to join our team. This role offers the opportunity to provide direct patient care, collaborate with primary care providers, and play a key role in shaping pharmacy services within our outpatient practices.
Job Description Summary The Ambulatory Care Clinical Pharmacist provides evidence-based pharmaceutical care to patients in the ambulatory clinical setting. The delivery of care by the Ambulatory Care Clinical Pharmacist includes, but is not limited to: designing, implementing, assessing, monitoring, and documenting therapeutic plans utilizing best evidence and most effective and most economical medication treatments; helping to achieve positive patient outcomes through interactions with patients, providers, and interdisciplinary teams in the ambulatory clinical setting. When appropriate, pharmacists who provide ambulatory care services may perform patient assessments, participate in collaborative practice agreements to provide medication management, provide collaborative drug therapy management, order, interpret, monitor medication therapy-related tests, coordinate care and other health services for wellness and prevention of disease, provide education to patients and caregivers, and document care plans in the medical record. Primary Locations: This position will initially focus on supporting three of our adult primary care practices:
- UVA Primary Care Riverside
- UVA Stuarts Draft Family Practice
- UVA Primary Care Waynesboro
Key Responsibilities:
- Provide medication therapy management and support chronic disease management programs, including Chronic Care Management (CCM), Remote Physiological Monitoring (RPM), Anticoagulation Management, Diabetes Management (glucometer placement, interpretation of results, and follow-up), and Hypertension Management.
- Conduct independent patient visits, with approximately 60% of time dedicated to direct patient care.
- Providers will place orders for services that the pharmacist will then render in alignment with the PCP's care plan.
- Document patient encounters and submit them for PCP review and signature.
- Establish relationships and build trust with primary care providers to integrate pharmacist services effectively within the clinics.
- Participate in the development of workflows and structures to enhance pharmacist involvement in patient care.
- Provide telehealth services as appropriate.
Work Schedule:
- Monday-Friday, 8:00 AM - 5:00 PM
- Opportunity for telehealth services is available.
Minimum Requirements
- Education: Bachelor's in Pharmacy with experience or Pharm.D. from an ACPE accredited College of Pharmacy. Degree required.
- Experience: PGY-1 residency required or equivalent clinical experience.
- License/Certification: Commonwealth of Virginia State Board of Pharmacy License required. American Heart Association (AHA) Health Care Provider BLS certification required. Board Certification or equivalent certification specific to clinical area highly encouraged.
Physical Demands Job requires sitting for prolonged periods; Repetitive motion: typing. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull < 20lbs. May be exposed to fumes, chemicals, blood/body fluids, and infectious disease.
