Responsible for assessment of resident psychosocial needs and coordination of support services for low-income older adults. The CMI also plans for and monitors services and interventions ensuring provision of quality care.
- Provides care management services to clients enrolled in the Assisted Living Waiver Program.
- Ensures all documentation complete and submitted in a timely manner in order to ensure enrollment in the Assisted Living Waiver Program for all those released from the waitlist.
- Partners with existing Care Managers during the transition and/or step down process.
- Completes Assisted Living Waiver Assessment Tool upon initial enrollment and every 6 months following for all assigned clients.
- Develops and initiates Individualized Service Plan upon initial enrollment and every 6 months following for all assigned clients.
- Ensures Medi-Cal remains active; completes re-certification with clients as needed.
- Conducts monthly telephonic contact with enrolled clients.
- Conducts client visits as clinically indicated.
- Maintains open, professional communication with California Department of Health Care Services staff.
- Establishes and maintains a care management relationship with clients and their informal support network as appropriate, offering respect, dignity and support.
- Documents via progress notes all case management activity regarding identified problems within 24-48 hours.
- Establishes and maintains open and effective communication with community providers, including physicians and other health care and social service workers.
- Utilizes supervision appropriately; maintaining open lines of communication and providing updates on caseload activity.
- Understands and applies the regulatory and procedural requirements of the Institute on Aging and the Assisted Living Waiver Program.
- All other reasonably related responsibilities as assigned.
EDUCATION: Bachelor's in human services field required. BACKGROUND AND EXPERIENCE:
- One year working with disabled adults and/or older adults required.
- Experience with and understanding of the medical and psychosocial problems of functionally impaired adults and older adults.
- Experience working with individuals with mental and/or behavioral health diagnoses and substance abuse disorders highly desired.
- Exceptional communication and presentation skills relating to functionally impaired adults and older adults, their support systems and teams of health professionals.
- Demonstrates case management skills and experience in the community health care delivery system.
- Detail oriented with good problem-solving skills and the ability to prioritize multiple tasks.
- Computer literacy required.
PHYSICAL & LANGUAGE REQUIREMENTS:
- Ability to lift up to 25 lbs.
- Ability to reach, bend, and walk
- Five finger manipulation
- Ability to speak and write English clearly
