Description
The Office of Population Health and Accountable Care (OPHAC) is focused on leading and facilitating the framework to improve population health outcomes across UCLA Health's Alternative Payment Model (APM) populations across the care continuum. As APMs evaluate outcomes regardless of where the care was rendered, OPHAC initiatives are targeted at improving population-based outcomes for care at UCLA Health and outside of UCLA Health (e.g.post-acute care settings, pharmacies). Successful participation in APMs is essential to respond to the evolving healthcare industry, as health systems are experiencing increased pressures to take on more financial risk and be held accountable for value-based outcomes. The Care at Home Program provides home-based primary care services and longitudinal palliative care to homebound and/or home-limited adults in UCLA Health's service area including greater Los Angeles County. The Care at Home Program will include comprehensive home visits to support patients and families with complex care needs, focus on understanding their goals and preferences for medical care, specialize in symptom management, and provide individualized care in managing chronic health conditions. Reporting directly to the Care at Home Program Nurse Practitioner Supervisor, Administrative Assistant III will direct support to the providers and patients for assigned primary and/or specialty care providers. Major duties include but are not limited to: acting as a liaison for the patients and providers participating in the Care at Home Program, assisting patients in accessing service within the health system, facilitating communication with referring physicians, referring patients to appropriate resources and providing follow-up through telephone and EPIC/Care Connect. Salary Range: $29.42-$42.14/hourly Qualifications
