MJHS Home Care provides advanced clinical services and emotional support to patients recovering from illness, injury or surgery. With a comprehensive range of services supported by an integrated health system, patients can recover in the familiar surroundings of their home.

  • The Quality Management Coordinator is an integral member of the Quality Management Departments and is responsible to implement, monitor and evaluate components of the Quality Management Program - including clinical care
  • The Quality Management Coordinator will serve as a team facilitator for improving performance in collaboration with the Director of Quality Management
  • Performs chart auditing and focus studies and assists Quality Management Leadership with the analysis and dissemination of data

  • Facilitates timely processing of home health care orders to ensure that they are compliant with regulations and are promptly sent to medical providers for review and signing
  • Reviews clinical documentation including Start of Care, Resumption of Care, Recertification, Transfer, and Discharge OASIS assessments and facilitates necessary corrections to ensure that documentation is accurate and compliant with regulations and agency standards
  • Participates in Quality Improvement activities in collaboration with management staff
  • Possesses understanding of regulatory bodies and standards related to delivery of services and educates staff regarding quality improvement, regulatory and accrediting body issues
  • Participates in and leads QAPI Committee and Root Cause Analysis meetings
  • Participates in Infection Control
  • Bachelor of Science in Nursing
  • 3 years prior experience in healthcare including at least 1 year in a Certified Home Health Agency (CHHA)
  • Microsoft Office Word, PowerPoint and Excel. OASIS certification (COS-C) preferred
