To provide quality pharmaceutical services to the patients, employees and medical staff of UCDHS. A Staff Pharmacist is an integral member of the health care team, responsible for drug therapy monitoring, distribution, drug information, and precepting pharmacy students and residents, conducting clinical research, participating in Department and Hospital Committees, task forces and work groups and fostering excellent interdepartmental
relationships.

Minimum Qualifications - For full consideration, applicants are encouraged to upload license and/or certification if required of the position.

• California Pharmacist License eligible and possess a valid license at the time of hire
• Doctor of Pharmacy Degree (PharmD), or equivalent training
• Post Graduate Year 1 (PGY1) Pharmacy Residency
• Experience in Ambulatory Care Clinic
• Experience in assessing lab data and appropriateness of drug therapy
• Experience in communicating patient care plans to healthcare professionals
• Experience in reviewing patient charts and creating medication management plans
• Experience with triage and assessing when a patient may be in distress and in need of medical attention
• Experience in identifying medication-related problems in patients, communicating them to a patient's physician and offering recommendations
• Familiarity with the use of physical assessment monitors such as OM self-management machines, asthma spacers/inhalers, blood pressure (BP) monitors, CoaguCheck machines, etc.

Preferred Qualifications

• Pharmacist Board Certification [e.g., Board Certified Pharmacotherapy Specialist (BCPS), Board Certified Ambulatory Care Pharmacist (BCACP), or Certified Specialty Pharmacist (CSP)].
• Post Graduate Year 2 (PGY2) Pharmacy Residency
• Experience working with and directing residents and students
• Knowledge of UCDH Pharmacy Services, policies and procedures

The Department of Pharmacy is a dynamic, integral, and critical part of patient care provided by UC Davis Health. As a department of greater than 600 professional, technical, and administrative support personnel, we focus on providing patient-centered medication management that optimizes outcomes through practice, education, research, and leadership through the continuum of care. We also have the largest pharmacy residency program in the country with 46 total residents in 13 different programs.

The Department of Pharmacy offers comprehensive pharmaceutical services (unit dose, IV admixtures, clinical services, infusion pumps, and teaching) throughout the health system complex 24 hours a day, 7 days a week. The pharmacy services encompass inpatient pharmacy services as well as an expansive footprint in the ambulatory care space where we have clinical pharmacists embedded within our clinics and specialty care locations. Our operational teams support the health system enterprise with three inpatient pharmacies, five outpatient pharmacies, two infusion pharmacies, a home infusion service line, and the full complement of pharmacy purchasing, stores, and distribution.
