Recently awarded among Arizona's Most Admired Companies in 2023 Seeking a Case Manager to join our interdisciplinary clinical team working with individuals in a justice-involved program. The Case Manager coordinates member referrals, provides case management services as needed, completes tasks that require serving the members who are enrolled in the Bridging the Gap Re-Entry /TIP Justice program. Full-Time Employed: 40 hours/week Location: 27th Ave and Indian School Rd. Bilingual in Spanish is a plus (Additional Language Differential Pay Available) Full Benefits Package, including 401K Generous PTO/Sick Time (4+ weeks in year 1) Duties may include:
- Responsible for direct face to face community-based services to assist client's re-entering into the community from jail and prison. Ensures linkage into community services and resources.
- Offers in-reach into jail, prison, sober living housing and treatment programs for the purpose of engaging member into BTG Re-Entry / TIP services. Orients potential clients to the BTG Re-Entry/TIP program.
- Provides transportation for members as appropriate and as medically necessary, and determined by the Re-Entry/ TIP team, in personal vehicle or in Terros Health provided vehicle, when available.
- Offers direct care wellness management and recovery strategies using strength-based engagement and psycho-social/educational /rehabilitation services as needed.
- Establishes therapeutic relationships with a variety of clients for the purpose of establishing and reaching appropriate treatment goals.

Qualifications
Education/Experience:
- High School Diploma or GED, as well as a combination of behavioral health education and work experience totaling a minimum of three (1) year; or an Associate's degree in any field, plus two years of work experience in a behavioral health service delivery. Bachelor's degree with 1 year experience or Master's degree in related behavioral health field.
- Computer Skills: Microsoft Word, Excel, Outlook, electronic medical record, NextGen preferred
- Minimum typing speed of 25 wpm
- Must have valid Arizona driver's license, be 21 years of age with minimum 4 years driving experience, and meet requirements of Terros Health's driving policy
- Must have a valid Arizona Fingerprint Clearance card (Level 1) or apply for an Arizona Fingerprint Clearance card within 7 working days of employment
- Interpersonal skills which allow for professional and positive relationships with co-workers, members, families, and other community members
- Good communication skills, evidenced by a non-judgmental approach, effective listening and positive interactions
- Certified Case Manager strongly preferred
- Must pass background check, TB test and other pre-employment screening
