Key Position Details:

Job Description:

Manages care coordination team that includes registered nurse care coordinators, social workers and care guides. Develops and implements strategies to integrate care across the continuum. Develops and maintains physician relationships, facilitating performance, quality initiatives and ongoing program development. Responsible for representing care coordination on system-wide committees and may lead system-wide initiatives and committees.

Principle Responsibilities

  • Maintains and improves operations by monitoring performance, identifying and resolving problems, completing audits and analyses and supporting continuous process improvement.
    • Oversees department activities with focus on quality, experience and effectiveness of service delivery.
    • Engages with interdisciplinary team regarding program and service planning, care coordination and process improvement initiatives.
    • Maintains awareness of financial performance and contributes to annual budget processes and other financial initiatives.
  • Develops and implements guidelines, workflows, policies and procedures that guide and support the provision of care coordination and social work.
    • Participates in evaluation, selection and integration of technology and information management systems.
  • Manages employees and holds them accountable for role responsibilities, competencies, individual performance and goals.
    • Supports recruitment, interviewing, hiring and onboarding process.
    • Conducts ongoing coaching, assessment, training and education of staff.
    • Completes and conducts employee performance reviews.
    • Responsible for engagement of employees and contributing to annual department wide engagement plan.
    • Maintains optimal staffing.
    • Maintains scheduling, time off approval and timecard sign-off.
  • Initiates and leads new projects and programs.
    • Identifies new opportunities for care coordination growth and expansion.
  • Other duties as assigned.

Required Qualifications

  • Bachelor's degree in nursing, social work or other related health care field
  • 5 to 7 years of clinical experience
  • 2 years of experience in a specialty
  • 0 to 2 years of management experience

Preferred Qualifications

  • Master's degree

Licenses/Certifications

  • Licensed Registered Nurse - MN Board of Nursing or Licensed Social Worker - MN Board of Social Work required upon hire only for applicants who hold licensure

Pay Range

Pay Range: $49.10 to $67.93 per hour The pay described reflects the base hiring pay range. Your starting rate would depend on a variety of factors including, but not limited to, your experience and education. Additional incentives may be available, including discretionary monetary bonuses, goal-based bonuses, and other financial incentives.
