PSH Housing Coordinator Compass Health is currently looking for a Housing Coordinator to join the team at Andy's Place in Everett! What you will be performing PSH Housing Coordinator is responsible for housing case management of assigned residents of Compass Health, Andy's Place. This position supports resident's housing stability and works closely with Compass Health clients, clinical staff, outside agencies, and community landlords to provide assistance with securing and maintaining housing for eligible Compass Health clients.
- Assists Compass Health clients with Shelter Plus Care, VASH, ALTSA or other subsidies in securing and maintaining housing in Broadway PSH.
- Provides direct encouragement and support for clients during the housing eligibility screening and lease up process including obtaining ID, income, chronic homelessness, and disability verifications.
- Assist new tenants in meeting their basic needs and establishing a routine of positive daily activities.
- Works closely with the Cascade Property Management and maintains open communication in order to resolve tenant or landlord issues.
- Attends initial housing subsidy program briefing with clients as well as any meetings related to lease violations and / or formal housing termination appeals.
- Provides housing support for individuals coming out of Western State Hospital.
- Coordinates care with VA, ALTSA, Compass Health and other clinical providers to facilitate the integration of individual care plans, goal setting, etc.
- Provides data and input for required reporting to various internal and external customers.
- Provide outreach, case management, crisis support, advocacy, and other needed services to residents, primarily in the community and residents' homes, but in any setting necessary, including streets, shelters, jails, hospitals, program office, etc.
- Meet or exceed the monthly client contact requirements of four times per month within the first three months of being housed and 3 times per month thereafter, including one home visit per month.
- Support residents' wellness and independence.
- Collaborate with other service providers in the community to ensure that residents are receiving high-quality, effective services to meet their needs.
- Maintain written and computerized records, compile reports and complete other program documentation (e.g. case notes, incident reports, home visit reports, monthly and quarterly reports). Maintain appropriate confidentiality of resident's information and records.
- Participate in after-hours on-call rotation.
- Actively look for ways to expand or change PSH program to better meet residents' needs.
- Maintains compliance with agency standards and regulations and acts in accordance with agency strategic plan and vision.
- Participates in regularly scheduled supervision and team meetings.
- Remains compliant with required trainings and certifications.
- Behaves in an ethical and professional manner consistent with agency policy, state law, and licensing entities if applicable.
- Understands and practices safekeeping of client protected health information per Compass Health's HIPAA and other agency policies and procedures.
- Demonstrates respect for diversity and a commitment to developing multicultural competency and sensitivity.

What you will be carrying along
- Bachelor's degree in social work, psychology, sociology, counseling or other related social service/science discipline. Or two years of experience providing case management services in lieu of education requirements. Certification and/or licensure in a relevant discipline (e.g., certified addictions counselor) may substitute for education requirement.
- 1+ year of experience working with people living with mental illness.
- Experience in residential services and/or supportive housing programs.
- Experience in medical, mental health or social services field preferred.
- Experience working in services for homelessness, mental health, substance abuse, preferred.
- Valid driver's license and ownership of a motor vehicle.
- Assist with the pickup and distribution of food donations from our community partners. This may include driving the agency van.
- Current First Aid & CPR certification and Food Handlers permit, or ability to obtain within 90 days of hire.
- Valid Washington State Driver's License required including vehicle and appropriate insurance, if applicable.
