Sentara is currently hiring an Integrated Care ManagerThis is apart-time position with day shift hours and great benefits! Position is located in Harrisonburg, VA.Job

This is a Continuum Case Management position focusing on the chronically and/or complex medically ill patient who may have experienced a lengthy hospital stay, have numerous comorbidities, multiple ED visits, various discharge planning needs, and/or a history/potential for readmissions. The continuum case manager follows patients beyond the walls of the hospital, into the community setting. Nurses makes home visits to perform medication reconciliation, make certain discharge instructions are clear to the patient and family, and set goals for chronic disease self-management. Follow-up visits and phone calls are scheduled with a frequency based on the patient's individual needs. The continuum case manager coordinates care with patient and family and partners with Community Health Workers to connect them to community resources, practitioners and agencies as deemed necessary.
