Essential Responsibilities:

The Population Health Quality & Data Assistant will have the following list of duties and tasks that may include, but not limited to,

1. Provide support to the Population Health team as needed and assigned.
2. Assist with the development of data quality and documentation reports.
3. Perform support tasks with data management workflows and processes.
4. Appropriate amendments to patient records in EPIC when instructed, with guidance from the team.
5. Process new data and information by updating databases.
6. Identify gaps such as missing values, inconsistencies, outliers, and duplicates.

Education/Experience

1. High school diploma or equivalent required

2. 1-2 years of experience with entering, auditing and monitoring data.
3. Excellent oral and written communication skills.
4. Be organized; able to set priorities and meet deadlines.
5. Proficient computer skills with Word and Excel
6. Must be able to work in a team environment and exhibit flexibility and enthusiasm in learning new information and developing new skills quickly.

Additional knowledge or experience of the following are preferred but not required:
* Healthcare clinical setting experience
* Understanding of healthcare terminology
* Experience using EPIC
