Description
POSITION: Placement Stability, Team Decision Making Facilitator

DEPARTMENT: Kansas Foster Care (Kansas City, KS)

Starting Salary: $48,551.30 - $53,961.22 (Annually)

(Starting salary depends on education and experience)

RESPONSIBILITIES: The Team Decision Making (TDM) Facilitator reports directly to the Director of Placement Stability, maintaining a high degree of autonomy in scheduling and facilitating Team Decision Meetings. The Team Decision Making (TDM) Facilitator ensures service compliance with contract/licensing/funding body/accreditation requirements. The Team Decision Making (TDM) Facilitator conducts structured TDM meetings on a flexible full-time basis for all placement related decisions, exercising caution to avoid unnecessary moves and disruptions for children in our care. The Team Decision Making (TDM) Facilitator will coordinate dialogue between the youth, case management team, members of the support team, placements, relatives, and connected adults to broaden the circle of community and relative support for all placements including relative, foster and group home. The Team Decision Making (TDM) Facilitator will strive to improve outcomes for child/ren and youth in foster care by helping build support networks to increase placement stability, reduce placement moves, expedite permanency, and help child/ren feel connected to and supported by their network.

QUALIFICATIONS: This position requires Bachelor's degree in Social Work or other Human service-related field (behavioral sciences field of study or education as deemed relevant by Cornerstones of Care). Supervisory-level (or above) OR two+ years of child welfare experience. Must be at least 21 years of age and pass background check, physical and drug screening. This position also requires a valid driver's license, proof of current vehicle insurance and reliable transportation.
