Routine tasks such as intake, triage, injections, and emergency care.
Build therapeutic relationships, assess chronic and acute health conditions (e.g., cardiovascular disease, diabetes, asthma), and address socio-ecological health barriers.
Develop treatment plans and link patients to appropriate support systems.
Work with the healthcare team (providers, medical assistants, social services) to ensure comprehensive patient care.
Help manage chronic illnesses and provide education on topics such as blood pressure, family planning, and immunizations.
Coordinate testing (e.g., glucometers) and ensure compliance with infection control protocols.
Ensure accurate and timely documentation of patient records, and adhere to regulatory requirements.
May oversee medical assistant staff, if required.
Facilitate home visits for patients unable to attend in-person appointments.
