The Registered Nurse - Patient Navigator will support complex pediatric patients in the Aerodigestive Clinic and the Program for Children with Medical Complexity by helping families address barriers preventing them from accessing care. The Patient Navigator will understand the clinical pathways established and will direct the patients to health care services within the organization, at outside facilities, and within the community for timely diagnosis, and treatment. This is not an oncologynursing position and does not require a certificate in oncology, as this position will support pediatric patientsonly. The ideal candidate will have 2-3 years of outpatient pediatric experience. Summary: The Registered Nurse - Patient Navigator - MHMG MMCP role guides patients through the care continuum, establishing contact with recurrent and newly diagnosed cancer patients and/or family members, or caregivers. The Patient Navigator will understand the clinical pathways established by the Oncology Program and will direct the patients to health care services within the organization, at outside facilities, and within the community for timely diagnosis, treatment and survivorship. Required Minimum Knowledge, Skills, and Abilities (KSAs)
- Education: Graduate from an accredited School of Nursing required. Bachelor's Degree in Nursing required or must complete BSN within three years of start date.
- License/Certifications: Current applicable state(s) license as a Registered Professional Nurse required. Oncology certification or defined process for achieving certification required.
- Experience: Experience working with oncology patients and families preferred. Experience in community outreach preferred.
