Overview

The Home Care Physical Therapist evaluates and develops treatment plans for patients of all ages, modifying the plan of care as appropriate. Serves as a role model in the delivery of professional services and as a clinical resource for staff and students. Hours may vary depending upon census and program need.
Qualifications

License/Certifications:

Licensed Physical Therapist (PT) within 90 days - Illinois Department of Financial and Professional Regulation (IDFPR)

Basic Life Support (BLS) within 30 days - American Heart Association (AHA)

Driver's License - Secretary of State (SOS)

Proof of Auto Insurance - Varies

Education:
Bachelor's Degree: Physical Therapy (Required)

Other Knowledge/Skills:

Ability to work as a member of an interdisciplinary team.

Drives reliable transportation and maintains state required insurance and valid Illinois Drivers License.


Responsibilities

Tests Performed: Performs only those CLIA tests that are authorized by the laboratory director and require a degree of skill commensurate with the individual's education, training or experience, and technical abilities.

Lab Procedures: Follow the laboratory's procedures for specimen handling and processing, test analysis, reporting and maintaining records of patient test results.

Record Maintenance: Maintains records that demonstrate that proficiency testing samples are tested in the same manner as patient samples.

Quality Control: Adhere to the laboratory's quality control policies, document all quality control activities, instrument and procedural calibrations and maintenance performed.

Corrective Action Policies: Follow the laboratory's established corrective action policies and procedures whenever test systems are not within the laboratory's established acceptable levels of performance.

Corrective Action Documentation: Document all corrective actions taken when test systems deviate from the laboratory's established performance specifications.

Problem Identification: Be capable of identifying problems that may adversely affect test performance or reporting of test results and either must correct the problems or immediately notify the technical consultant, clinical consultant or director.

Department Specific Job Function

Completes comprehensive OASIS assessment of patients.

Assess homebound status of patients.

Completes comprehensive physical therapy assessment in patient's home and develop plan of care.

Communicate with patients to schedule appointment times and manage daily schedule.

Assess home safety and recommend equipment to improve patient's home safety.

Educate family/caregivers on recommended home exercise program.

Performs wound care within the scope of practice and according to the plan of care and to the department policy.

Compensation and Benefits

The compensation range for this position is $34.66per hour - $59.62per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate's experience, qualifications, location, training, licenses, shifts worked and compensation model.
