POSITION SUMMARY
The Palliative Care Coordinator is a registered nurse who is responsible for program development, management, and growth of the palliative care program. They collaborate with internal and external partners and service providers to assure the clinical, human resource, financial, quality, and planning needs to allow for an efficient, responsive, and compliant Palliative Care Service. Coordination of care begins with the patient at diagnosis of serious illness and remains throughout the entire diagnostic and treatment process, survivorship and/or end of life. This role is responsible for ongoing development of the program, assessing, providing patient information, providing support to the patients and their families, and ensuring continuity of care for patients appropriate for palliative care. They also facilitate regular team meetings and coordinate with the palliative team to meet patient needs. This role will make visits where appropriate such as in the patient's home/residence, inpatient, or outpatient settings.
POSITION QUALIFICATIONS
ESSENTIAL RESPONSIBILITIES
- Resource Management: Coordinate Palliative Care services to operate in conjunction with the organizational strategic priorities.
- Ensure that systems are in place to support decreasing hospital length of stay and readmissions for palliative care patients.
- Support a culture of continuous improvement through quality monitoring, regulatory compliance, risk management and performance improvement activities.
- Support the ongoing development of palliative care program.
- Support the development and implementation of policies/procedures/standards of care to meet Joint Commission, state and federal rules and regulations.
- Participate in the measurement and evaluation of care processes to identify variations and opportunities for improvement.
- Act as a clinical resource for patients/families, interdisciplinary teams, and direct care providers.
- Serve as a liaison between physicians and departments to ensure timely and quality care deliver. Guide care delivery by all care providers.
- Meet newly referred palliative care patients to assess needs and begin education and support of treatment plan and coordination of appointments/services.
- Identify high-risk patients based on protocol guidelines.
- Meet with chronic/focused care management patients to assess needs, develop a plan of care, and coordinate appointments and services.
- Develop and maintain patient care plans.
- Consistently demonstrate putting the patient at the center of their care. Involve patients in decisions regarding their individual preferences and review medical options in an easy-to-understand vocabulary.
- Serve as a patient advocate and remain available to respond to patient and staff questions or refer to appropriate resources.
- Build and maintain relationships with community programs and resources.
- Refer to appropriate internal and external resources as needed.
- Plan for and provide patient, family, caregiver education by identifying needs and communicating information clearly.
- Prepare and provide education for staff on subjects related to the palliative care program and chronic/focused care management program
- Develop, revise, and maintain patient education materials.
- Conduct seminars and support groups to meet the educational and emotional needs of patients. This includes public speaking at community and CRMC events.
- Provide clinical data to insurance representatives to advocate for necessary treatment of patients.
- Schedule and oversee interdisciplinary team meetings.
- Attend workshops and read current literature for self-education and improvement.
- Communication: Develop and maintain effective working relationship with staff, other departments, system administration/leadership and community organizations to promote positive image, heighten awareness, and promote collaboration.
- Represent CRMC and Palliative Care at required meetings, boards, conferences, and community/system events.
- Assure support for palliative consultations for clinic, community, and hospitalized patients needing formal Palliative Care appointments and services.
- Exercise concern for the patient's welfare, maintain knowledge of the patient's condition, work to improve or maintain the patient's quality of life, and support services to reduce suffering.
- Ensure compliance and quality service with JCAHO, state and federal rules and regulation through provision of client care, policies, procedures, and documentation.
- Remain up to date on new skills and technology, utilizing nursing process (observation, assessment, planning, implementation, and evaluation) in the delivery of care.
- Perform duties in accordance with physician's order, organizational policies, methods, and techniques, and in coordinating with other departments and Medical Center personnel.
- Demonstrates Standards of Excellence when other duties are assigned.
