Description

Provide thoughtful and effective services as an IRIS Consultant (IRIS = Include, Respect, I Self-Direct) to individuals directing their long-term care, supports and services. As a member of an ICA Team, partner with individuals receiving long-term care supports and services to understand and develop what is possible and desirable in their lives. Work in a Social Services capacity assisting Participants with maintaining their autonomy and independence and monitoring for health and safety.

This is full-time (40 hours/week), benefit eligible, hybrid opportunity with hours between 8am-5pm with travel in and around Milwaukee County and surrounding areas.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

This list of duties and responsibilities is not all-inclusive and may be expanded to include other duties and responsibilities as identified by the ICA Team.



  • Develops trusting relationships with individuals served by the ICA.
  • Assists individuals to explore the ways in which IRIS can enable them to receive supports and services in their community to lead more fulfilling lives.
  • Provides support and assistance to Participants to help them understand the IRIS Program and maintain eligibility for services.
  • Works with Participants in the IRIS Program and the team the Participant chooses to create the required IRIS Support and Services Plan.
  • Continually seeks to learn and to share better ways to enable those we serve to lead more fulfilling lives.
  • Works as part of an ICA team to participate in hiring, peer mentoring, learning, and mutual support to others within the team.
  • Connecting with local agencies and resources to build relationships that will help provide service options to participants and increase referrals; including relationships with ADRC's. Maintains required documentation to assure state requirements for quality markers are met in a manner that meets and exceed program requirements.


    • Monthly contact and quarterly visits must be completed with all participants
    • Additional contact (phone or in-person) may be required based on participant need and contractual requirements (ie. 40 HHS, VHR, etc.)
    • All contact with participants, guardians, lateral contacts must be documents in required systems within 2 business days of the contact.
    • Additional documentation will be required for annual visits and quarterly visits
    • All annual visits must be documented in all required systems
    • Must maintain a tracker with all participant documentation and visits
    • All files must be in compliance with IRIS requirements and will be regularly reviewed internally and by external partners.


  • May conduct annual Long-Term Care Functional Screen
  • Work and learn independently.

QUALIFICATIONS

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Upon offer, candidates must successfully complete the necessary background, caregiver, medical and any other checks required, according to program requirements. EDUCATION AND/OR EXPERIENCE:



  • Possess a minimum of a Bachelor's Degree in social work, psychology, human services, counseling, nursing, special education or a related field and have a minimum of one year of supervised experience working with seniors and/or people living with disabilities; OR

    • In lieu of degree, have a minimum of four years of direct experience related to the delivery of social services to seniors and/or people living with disabilities and long-term care needs in community settings; OR
    • Be a current IRIS Consultant in good standing who has received and exemption from meeting these criteria by the Wisconsin Department of Health Services.

  • Capacity to use and apply smartphone, tablet and related technology.
  • Computer skills are also required, as operation of certain software programs will be necessary.
  • Considerable experience and knowledge of human behavior, dynamics of groups, interpersonal relations, negotiation and social interaction.
  • Willing to serve participants regardless of race, ethnicity, demographic, and culture in the participants community.
  • Ability to organize and plan work effectively and strong organizational skills are required.
  • Ability to establish and maintain effective working relationships with community partners.


CERTIFICATES, LICENSES, REGISTRATIONS:

The incumbent of this position must have a valid driver's license and have reliable transportation to perform the essential duties of the role; a motor vehicle check (MVR) with a satisfactory driving record per the LSS Driver Safety Procedure is required, and ability to meet LSS auto insurance requirements.

TRAVEL: Ability to travel on day trips as required up to 30-50%.
