Position Summary
The Program Coordinator (Case Manager, Healthy Start) are staff of the Office of Community Engagement & Neighborhood Health Partnerships (OCEANHP) at the University of Illinois employed through the Healthy Start Grant. The Case Manager facilitates and encourages adequate prenatal, inter-conception, and pediatric health care utilized among Healthy Start program participants and their children.

Duties & Responsibilities

  • Closely coordinates services for clients with medical personnel.
  • Builds working relationships with prenatal, inter-conception, and pediatric clients through caring, support, and empathy.
  • Develops a consistent schedule to visit with participants with frequency determined by risk level.
  • Provides home-based services to women and infants/children to provide health education, breastfeeding/lactation support, doula services, and other case management-related services.
  • Screens and refers for substance abuse and postpartum depression treatment as indicated.
  • Teaches inter-conception program participants about family planning.
  • Teaches prenatal clients about child development and skills to enhance child development and provides screening according to programmatic guidelines.
  • Works collaboratively with Healthy Start outreach/educators and other community medical providers, and Advance Practice Nurses.
  • Links participants with educational, training, and employment programs.
  • Informs the Healthy Start Team of resources available to community families.
  • Maintain family records as required by the agency and program standards.
  • Maintains accurate and timely records, including designated information on participants, and produces reports on all program activities as assigned. Collaborates with Community Resources for the benefit of client families and the program, refers, and advocates for participant families at appropriate community resources.
  • Maintains professional competence through on-going trainings and professional development opportunities.
  • Provides data and information that will assist in program reporting.
  • Collaborates with Healthy Start Team Members to meeting the Program's Qualitative and Quantitative Goals.
  • Provides birthing support education and guidance to pregnant women for first six weeks to post-delivery.
  • Perform other related duties and participate in special projects as assigned.


  • Minimum Qualifications

    1. Bachelor's degree.
    2. A total of one (1) year (12 months) in education, training and/or work experience in the area of specialization inherent to the position. (Note: Master's Degree in an area consistent with the duties of the position may be substituted for one (1) year of work experience.)
    3. Experience in working with families of diverse backgrounds and able to manage and maintain an active case load.


    This position requires Illinois residency within 180 days of the hire date.

    Preferred Qualifications
    • Experience working with parents and families.
