Position Summary: We have an exciting opportunity to join our team as a Community Health Rep. As a critical member of the research and healthcare team the Community Health Rep will serve as an important liaison between the community and healthcare providers for individuals with kidney disease The CHR will assist patients with overcoming challenges encourage self advocacy and motivate patients to remain engaged in their clinical care Additionally the CHR will help patients identify and navigate clinical and community resources such as resources for food e g Supplemental Nutrition Assistance Program SNAP WIC food pantries ; resolving housing quality issues e g lack of heat/hot water mold and pests ; finding and applying for affordable housing; preventing utility shut offs and evictions; and connecting with other members of the healthcare team or healthcare services. Job Responsibilities:
Minimum Qualifications: To qualify you must have a High School Diploma and minimum of 1 year related experience in an office setting or the equivalent combination of education and experience is required Effective Oral written communication and interpersonal skills Preferred Qualifications:

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $48,274.10 - $52,000.00 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need.
