GENERAL SUMMARY:

Responsible for providing cardiovascular care services, as delegated by the physicians, including assessing and managing patients in the hospital following established standards and practices. This position covers both hospital and clinic coverage.

PRINCIPAL JOB FUNCTIONS:

1. *Commits to the mission, vision, beliefs and consistently demonstrates our core values.

2. *Performs and documents medical history and physical examinations on cardiovascular patients; establishes presumptive diagnosis, which are to be countersigned by the responsible physician; reviews patient records.

3. *Makes complete chart entries on patient transaction, writes orders and records progress notes, which will be reviewed and countersigned by the responsible physician within 24 hours.

4. *Available for consultative visits with the patient and their family; serve as a resource person to the staff and physician; recommends changes in plan of care to facilitate optimum patient outcomes.

5. * Interprets and integrates data to determine appropriate diagnostic and therapeutic procedures as needed; alert physician to test results requiring their attention.

6. * Orders appropriate laboratory and diagnostic procedures; keeps physician informed of changes requiring intervention.

7. *Synthesizes data to determine preliminary diagnosis and therapeutic plan utilizing principles of prevention.

8. * Triages patient and provider telephone calls and provides consultations; may request consultation at the direction of the sponsoring physician.

9. *Prepares and completes patient discharge summaries of patient hospital care on the day of discharge or within 14 days of discharge.

10. *Participates in management of ICU, Medical and surgical emergencies, utilizing ACLS protocol.

11. *Performs independent evaluations and treatment procedures in emergencies; teaches/counsels individuals regarding follow-up care and activities as needed.

12. *Provides monitoring and continuity of care between physician visits.

13. *Monitors exercise and pharmacologic stress testing (Day APPs only).

14. *Complies with OSHA clinical safety practices as well as administrative and facility regulations.

15. Remains current in advances in cardiovascular care and, where relevant, translates this knowledge into clinical practice.

16. *Participates in continuing education and maintains professional certifications and affiliation.

17. Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise.

18. Participates in meetings, committees and department projects as assigned.

19. Performs other related projects and duties as assigned.

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:

1. Maintains clinical competency as required for the unit including but not limited to age-specific competencies relative to patient's growth and developmental needs, annual skill competency verification and mandatory education and competencies.

2. Knowledge of computer hardware equipment and software applications relevant to work functions.

3. Knowledge of local, state and federal regulations related to nursing and medical services within practice area.

4. Knowledge of care of the critical and complex patient and their medications possible side effects on patients with co-morbidities.

5. Knowledge of process improvement and change management principles within the healthcare environment.

6. Knowledge of professional regulations and practices in giving and evaluating patient care.

7. Knowledge of hospital safety hazards and precautions.

8. Ability to perform physician exam of the complex patient.

9. Ability to evaluate the patient treatment needs by reviewing past medical/surgical history, performing physical exam and required follow up exams.

10. Ability to incorporate diverse teaching strategies relating to documentation requirements to achieve quality management, clinical and operational goals.

11. Ability to facilitate change and manage projects through analysis, key end user engagement, and consensus building within an interdisciplinary environment.

12. Ability to effectively organize and prioritize projects managing time working with minimal supervision.

13. Ability to assess, plan, implement and evaluate the clinical theoretical processes as they relate to patient care.

14. Ability to problem solve and engage independent critical thinking skills.

15. Ability to work independently, take direction and work collaboratively with other health team members from diverse backgrounds.

16. Ability to communicate effectively both verbally and in writing.

17. Ability to establish and maintain effective working relationships with all levels of personnel and medical staff.

18. Ability to maintain confidentiality relevant to sensitive information.

19. Ability to prioritize work demands and work with minimal supervision.

20. Ability to maintain regular and punctual attendance.

EDUCATION AND EXPERIENCE:

Master of Science degree in Nursing with current Advance Practice Registered Nurse licensure from the State of Nebraska required. Certification as an Adult Nurse Practitioner, Family Nurse Practitioner or other related specialty area required, OR, Master of Science in Physician Assistant & certification through AAPA and current State of Nebraska Physician Assistant license required. Certification with the U.S. Drug Enforcement Administration for prescriptive authority required. Minimum of three (3) years clinical practice experience preferred.
