Case Manager - Castleview

Position Summary

The Salvation Army Northern Division Rochester Castleview Case Manager is responsible for providing a full range of case management services to Castleview residents.

$25.00 - $26.00/hour, 40 hours/week

Essential Functions (approximate time spent)

• Provide long-term strength-based, person-centered holistic case management to Castleview residents. (60%)
• Coordinate services for participants with collaborative agencies. (15%)
• Assist with program fiscal responsibility including budgeting, grant spending, and billings. (15%)
• Maintain up-to-date, accurate participant information in casefiles and through database entry. (5%)
• Engage in internal and external activities/meetings to promote and further The Salvation Army goals and objectives. (5%)

Education and Experience

• Bachelor's degree in relevant field. In lieu of degree, education and experience may be considered.
• If degree is in social work, must be licensed by state board.
• Experience working in a social service setting conducting long-term case management preferred Must pass background checks, possess valid driver's license, and meet MVR requirements.

Working Conditions

• A variety of work including sitting, standing, walking, reaching, bending, typing, and lifting.
• Professional environment; not eligible for a remote-friendly work environment.
• Must be able to communicate clearly in English.
• Standard business hours with occasional evenings and weekends.
• Must be able to occasionally lift 20 pounds.

Supervisory Responsibilities

None.

Travel/Driving Requirements

Drive agency vehicle locally to facilitate case management services.
